- If you are injured at work, and it is a life-threatening situation, immediately call 911.
- For non-life-threatening injury, seek medical attention and inform the caregiver that you were injured on the job, and that the bill can be sent to Harvard's Disability Claims Unit, Holyoke Center Room 640.
- In either case, you must report the injury to Kim (496-2231), Jessica (495-5636), or your center time approver as soon as possible.
- Print an Accident Report on yellow paper and fill in the section for Injured Employee, even if the injury was not significant. If you cannot return to work, Selena or your center human resources officer can fill in the form for you, and mail it to you for your signature.
- Send the form to Selena or the center human resources officer at 124 Mt. Auburn Street, Suite 101.
