SOCIOLOGY & SOCIAL POLICY FORMS

Oral Exam Forms
Petition to appoint an oral exam committee and form for report of the oral exam results.

From the oral exam stage onward, all paperwork should be submitted directly to the graduate coordinator in Social Policy, for consideration by the Committee on Higher Degrees in Social Policy. To ensure up-to-date information in the disciplinary department, students should submit duplicate copies of this paperwork to the Sociology graduate coordinator.

Dissertation Prospectus Forms
Petition to form a dissertation committee and report of the dissertation prospectus defense.

These forms should be submitted directly to the graduate coordinator in Social Policy, for consideration by the Committee on Higher Degrees in Social Policy. This information will be shared with the Sociology Department coordinator, although students may find it helpful to submit copies of all documentation to Sociology as well.

New phone number for degree-related questions: The Registrar's Graduate Requirements Office has established a new phone number for GSAS students with degree questions (only): 496-4182.

 

Application for degree form (pdf)
The completed and signed application for degree form is due at the Registrar's Office by Aug 10, 2009 (for Nov degree), Nov 30, 2009 (for March degree), or Mar 26, 2010 (for May degree). Please allot time to obtain Chair's signature.

Candidates for the A.M. degree should have the form signed by the Chair in Government or Sociology, as the A.M. degree is awarded in the disciplinary field.

Candidates for the Ph.D. should seek the signature of the Chair in Social Policy by submitting the form to Pamela Metz. The correct wording for the subject field is "Government and Social Policy" or "Sociology and Social Policy". The Department/Division/Committee should read "Social Policy." In listing courses offered for the Ph.D., students may count Time-R or Time-C toward the 16 half-course requirement if needed.

Upon filing the application for the Ph.D., Social Policy students should alert Pam Metz to their expected timetable for completion of the dissertation and its defense. This will help ensure that all forms and procedural requirements are in place for the degree to be awarded on schedule.

 

Reactivation of an application for degree (pdf)
If a student does not receive the degree on the date for which he or she originally applied, the student must submit a reactivation of degree application form. Students may reactivate a degree application once without charge; for any subsequent reactivation, the fee is $50.

Form of the Ph.D. Dissertation
Detailed GSAS rule booklet concerning the required form of the dissertation and degree submission procedures. Pay special attention to the formatting and submission checklists on pages 16-17.

(Note!) The critical formatting issue for Social Policy students is that the dissertation title page list the degree correctly. The official program name is "Committee on Higher Degrees in Social Policy". The official subject name is "Government and Social Policy" or "Sociology and Social Policy."

 

 

Ph.D. packet (forms due upon submission of dissertation)
Students are advised to fill out these forms before bringing their dissertation to the registrar. They reportedly take about 20 minutes to complete. If students bring completed paperwork with them, the submission process will be much simpler.
