Abstract
The U.S. health care system has long struggled to deliver high-quality care at a reasonable cost. Though there is plenty of blame to go around, fingers are often pointed at health insurers. These intermediaries are comparatively easy targets; they aren’t beloved clinicians delivering lifesaving care or (less beloved but still revered) biopharmaceutical companies creating wonder drugs.
